This is a new old stock pair of 12AT7 vacuum tubes made by General Electric. Both have an April, 1964 date code on the glass. These are fully tested and matched within 5% Gm on our Hickok USM118B Card

 

I think I got these out of TVs though I might be wrong. I have a third like this with a transformer mounted to it. Wires were cut off the terminals and will have to be desoldered.
